Ultherapy Prime Maintenance: How Often Should You Repeat Ultherapy Treatment?

You’ve done your Ultherapy PRIME treatment.
You love the lift, you notice your skin feels firmer, smoother, more supported.
Your jawline looks cleaner. Your brows feel subtly lifted. Your face just looks… put together.
And then, a thought quietly forms:
“I think I’ll do this again.”
If that sounds familiar, you’re not alone.
At Ozhean Zoey, this is one of the most common feedback lines we hear from patients after their Ultherapy PRIME journey. Many don’t come back because something has “dropped badly” again. They come back because they like how their face feels and looks with Ultherapy PRIME in their life.
This is not unique to Singapore, nor to Ozhean Zoey alone. Across our Ozhean centres globally, Ultherapy PRIME has become a treatment patients consciously choose to repeat. Not out of urgency, but out of confidence.
So how do you know when to plan your next Ultherapy PRIME treatment?
There is no single fixed answer, and that’s actually a good thing.
Planning your next Ultherapy PRIME session should never be calendar-based alone. Instead, it should be guided by these key considerations, starting with the most important one.
1. How satisfied are you with your initial treatment?
This sounds simple, but it’s often overlooked.
Ask yourself honestly:
- Are you happy with the level of lift?
- Do you feel the results are subtle, moderate, or exactly what you hoped for?
- Do you wish certain areas were tighter or more defined?
In a separate article, we explain the average and recommended number of Ultherapy PRIME shots for different areas of the face and neck. These shot numbers are based on anatomical studies, clinical experience, and Merz-guided protocols.
That said, numbers alone do not define outcomes.
Every Ultherapy PRIME treatment at Ozhean Zoey is still assessed, planned, and customised by our doctors to optimise lifting, facial contouring, and overall skin quality improvement for you.
What if you intentionally chose a lower number of shots?
Some patients choose a more conservative approach during their first Ultherapy PRIME treatment because they want to “try and see”, prefer very subtle change, or they are just easing into non-surgical lifting.
If you later feel the results are too subtle for your liking, this does not mean Ultherapy PRIME “didn’t work”.
It simply means you may not have reached your optimal treatment intensity yet.
In such cases, it may be appropriate to discuss a follow-up enhancement treatment with your aesthetic doctor.
Timing matters here
Ultherapy PRIME works by stimulating your body’s own collagen regeneration. This process takes time.
Optimal results are typically seen around 3 months post-treatment.
If, at the 3-month mark, you feel certain areas could benefit from more lifting, slimming, or tightening, then a targeted follow-up session to top up shots to optimal levels can be discussed.
This is an Ultherapy enhancement phase, not Ultherapy maintenance yet.
2. Clarifying your treatment goals: lift, slim, refine… or all three?
The next key factor in planning your Ultherapy PRIME timeline is understanding what you are targeting.
Ultherapy PRIME can address multiple concerns, but different goals may require different planning.
Lifting
- Jawline definition
- Brow elevation
- Mid-face support
- Neck tightening
Slimming and contouring
- Lower face heaviness
- Jowl fullness
- Submental (under-chin) bulk
Skin refinement
- Improved firmness
- Better skin texture
- Overall “tightened” skin quality
Merz Aesthetics recognises that while many patients need only one treatment, some benefit from additional treatments depending on laxity and biological response.
If your doctor assesses you at 6 months and finds that certain areas could benefit from further lifting or slimming, it may be reasonable to plan a follow-up treatment at that point to further enhance results.
Again, this is not Ultherapy maintenance. This is part of achieving your desired endpoint.
3. Follow-up enhancement vs maintenance: an important distinction
This is where many patients get confused, so let’s be very clear.
Follow-up Ultherapy enhancement treatments are usually considered 3–6 months after the initial treatment. They are typically done when results are good but not yet optimal, or certain areas need additional support.
The primary purpose is to further enhance the non-surgical facelift effect.
Think of this as finishing the sculpture.
Ultherapy maintenance treatments on the other hand means something very specific.
By definition, a maintenance Ultherapy PRIME treatment is done only after:
- you have achieved a satisfactory level of:
- facial contouring,
- lifting,
- brow elevation,
- facial slimming,
- and neck tightening,
- facial contouring,
- and you now wish to maintain these results over time as aging continues.
Ultherapy maintenance is about preservation, not correction.
This distinction is critical, and one that experienced aesthetic doctors take seriously.
4. When does Ultherapy maintenance usually begin?
Once optimal results are achieved and sustained, maintenance treatments are planned to help you keep pace with natural aging.
Official Merz guidance supports the concept that while many patients require only one treatment, future touch-up or Ultherapy maintenance treatments can help maintain results as the skin continues to age.
In clinical practice, maintenance is most commonly considered around 9–12 months or 12–18 months, depending on individual aging patterns and goals.
If your doctor assesses you earlier and finds results are still holding well, maintenance can be delayed.
If changes are appearing sooner, maintenance may be planned earlier.
5. Special note for more severe laxity
For faces with more advanced laxity, Ultherapy PRIME alone may not always achieve maximal lifting.
In such cases, we often recommend considering OZ PCL filler lift protocols in combination.
This can:
- boost lifting effects by approximately 30–50%,
- provide additional structural support,
- and help Ultherapy results hold well over time.
This is not about doing “more treatments”, but about doing the right combination for your anatomy.
6. Recommended maintenance schedules by age group
While every face is unique, age does influence collagen production and skin behaviour. A general framework many patients find helpful:
In your 30s
- Goal: prevention and preservation
- Maintenance: every 18–24 months, or once yearly for proactive patients
In your 40s
- Goal: lift, hold, and slow progression
- Maintenance: every 12–18 months
In your 50s and beyond
- Goal: sustained structural support
- Maintenance: often once yearly
- Combination approaches are more commonly considered
It is important to note that these are guidelines, not standard rules to follow blindly.
7. Factors that affect how often you need maintenance
Several factors influence how long your Ultherapy PRIME results last:
- Baseline skin laxity
- Age and collagen response
- Sun exposure and lifestyle
- Weight fluctuations
- Smoking and metabolic factors
- Areas treated (face vs neck)
- Your aesthetic expectations
This is why maintenance planning should always be personalised.
A Smart Ultherapy PRIME Maintenance Plan
A thoughtful, doctor-led plan usually looks like this:
- Initial Ultherapy PRIME treatment
- Assessment at 3–6 months
- consider enhancement only if needed
- consider enhancement only if needed
- Achieve optimal lifting and contouring
- Maintenance planning at 9–18 months
- Combination treatments added only when indicated
This approach avoids overtreatment and ensures natural, sustainable results.
Frequently Asked Questions
Here are some commonly asked questions we get:
How long do Ultherapy PRIME results last?
Results can last 12–18 months or longer, depending on individual collagen response and aging.
Can Ultherapy PRIME be done once a year?
Yes. Many patients choose a yearly maintenance rhythm to maintain consistent results.
Is it normal to want a second treatment?
Yes. Especially if your first treatment was conservative or if certain areas need further enhancement.
Is maintenance the same as follow-up treatment?
No. Follow-up treatments enhance results. Maintenance preserves achieved results.
Final Thoughts: Maintenance Is About Confidence, Not Correction
Ultherapy PRIME maintenance is not about chasing youth. It’s about maintaining structure, definition, and skin quality in a way that feels natural and sustainable.
When done thoughtfully, it becomes less of a “treatment” and more of a long-term skin strategy.
If you’re considering your next Ultherapy PRIME step, our doctors at Ozhean Zoey will be happy to assess where you are in your journey and guide you on whether enhancement, maintenance, or combination treatments are most appropriate.
