Re20 vs Re20 Fine in Singapore: Which ECM Skin Booster Do You Need for Eyes, Lips & Face?

August 4, 2026
Re20 and Re20 Fine are the same ECM skin booster in two particle sizes — here's how to choose the right one for your under-eyes, lips and face, and where the technology is heading next. Most people meet Re20 as a single, striking idea — “injecting real skin into skin.” What fewer realise is that it comes in two forms, and choosing the wrong one is the difference between a smooth, natural result and a lumpy one you notice every time you smile. The decision isn't about which version is “stronger.” It comes down to particle size — and matching that particle size to the skin you're treating. Re20 is an ECM (extracellular matrix) skin booster — a category barely two years old, built on human-derived matrix rather than synthetic gel. Re20 Fine is its precision sibling, micronised specifically for the thinnest, most mobile skin on your face: the under-eyes and lips. Placed correctly, each does something conventional boosters and standard fillers simply can't. This guide explains the difference and shows you exactly which one your eyes, lips and face need, and looks at where ECM technology — including the forthcoming Re20 Volume — is taking anti-ageing next. What is an ECM skin booster? An ECM skin booster is an injectable made from real, human-derived extracellular matrix — the collagen, elastin and support proteins that form your skin's scaffold — with the living cells removed. Rather than only hydrating or stimulating, it replenishes the skin's structural framework directly, giving your own repair cells the environment they need to rebuild. To see why that matters, it helps to know what other injectables do. Within the broad skin booster category (水光针, shuǐ guāng zhēn), a hyaluronic acid (HA) booster hydrates — it holds water like a sponge. A polynucleotide (PN) booster repairs and calms. A biostimulator such as PDLLA or PCL stimulates your cells to lay down more collagen over time. An ECM booster does something more fundamental: it rebuilds the framework all of those things live inside. Think of your skin as a garden. Fillers water the plants. Biostimulators tell the plants to grow. But if the soil itself is depleted, no amount of watering or encouragement helps for long. The extracellular matrix is that soil — and Re20 replenishes it with real matrix material derived from human acellular dermal matrix (hADM): purified, decellularised skin scaffold made of collagen, elastin, proteoglycans and growth factors. This isn't just marketing framing. A 2020 review in Plastic and Aesthetic Research on the role of the ECM in skin ageing describes the matrix as central to tissue homeostasis, and research on fibroblast–ECM interactions shows a self-perpetuating problem: as matrix fragments with age, fibroblasts lose their grip, produce less new matrix and more matrix-degrading enzymes, which fragments the scaffold further. Replenishing the ECM aims to interrupt that cycle at its root. A 2025 pilot study quantifying collagen, elastin and glycosaminoglycans across the facial dermis — including the under-eye region — confirms just how much these structural proteins decline with age, and how thin that under-eye tissue really is. Re20 vs Re20 Fine: the particle size difference Re20 and Re20 Fine are made from the same human-derived ECM material. The single most important difference is particle size. Re20 is refined to roughly 75µm for the full face and neck. Re20 Fine is micronised further, to roughly 50µm, for the delicate, mobile skin of the under-eyes and lips. A useful way to picture it: same premium coffee beans, different grind. A coarse grind is right for a French press — a large, forgiving vessel. A fine grind is what you need for delicate espresso, where anything coarser ruins the result. Re20 is the coarse grind for the broad canvas of the face; Re20 Fine is the fine grind for the precise, unforgiving zones around the eyes and mouth. Feature Re20 (Classic) Re20 Fine Material Human-derived ECM (hADM) Same human-derived ECM (hADM) Particle size ≈ 75µm ≈ 50µm — ultra-fine Reconstitution Over 5cc 1.5–2cc Recommended areas Full face, cheeks, neck Under-eyes, lips, thin/mobile skin Targets Density, tone, pores, elasticity Hollows, dark circles, fine lines Role Overall skin foundation Precision renewal for delicate zones Particle-size specifications reflect the manufacturer's (L&C Bio) product brochure. Some third-party sources quote a different figure for classic Re20 — always confirm the current spec with your treating doctor. Why particle size matters so much on thin, moving skin This is the heart of the whole decision — and the reason you genuinely need Re20 Fine, not classic Re20, around the eyes and lips. Under-eye skin is the thinnest on the body — often less than half a millimetre. Picture spreading gravel under a sheet of tissue paper versus spreading fine sand. The gravel shows every lump; the fine sand lies flat and invisible. Inject a coarser particle into paper-thin under-eye skin and you risk exactly that: visible bumps (embossing), a bluish Tyndall tint showing through, and easier bruising. Micronised to ~50µm, Re20 Fine passes through delicate tissue with far less resistance — so it settles smoothly, swells less, and recovers faster. The lips add a second challenge: constant movement. A road that flexes will crack any rigid cobblestone surface laid on top, but fine, well-graded material moves with it. In a high-movement zone, coarser or more rigid product is what creates the dreaded bulge when you smile. Finer particles integrate into the tissue and move as your face moves — which is precisely why Re20 Fine is the right tool for lips and the smiling under-eye, and classic Re20 is not. Which do you need? A simple guide by area Match the grind to the ground. As a rule of thumb: • Under-eyes, tear troughs, dark circles — Re20 Fine. Thin, mobile skin demands the ultra-fine grind. • Lips (hydration, fine lip lines, subtle natural volume) — Re20 Fine. Movement plus delicacy again point to Fine. • Smile lines and perioral fine lines — Re20 Fine for skin-quality renewal (deeper folds may be combined with a volumiser). • Full face, cheeks, overall skin density, tone and pores — Re20 (Classic). The broad canvas suits the standard grind. • Neck — either; Re20 Fine for the thinnest, crêpey areas, Re20 for broader treatment. In short: if the skin is thin or moves a lot, choose Re20 Fine. If you're renewing the wider face, classic Re20 is built for the job. Your doctor confirms the right choice after assessing your anatomy. Re20 Fine for lips and smile lines The lips and the skin framing them are thin, mobile and quick to show age — the exact brief Re20 Fine was designed for. Here, it works in three ways rather than simply parking a bolus of gel on top. Freshens and hydrates. By replenishing the lip's own matrix, Re20 Fine supports a naturally fuller, better-hydrated, healthier-looking surface — the “quiet” plump that comes from within rather than from overfilling. Subtle, natural augmentation. It restores gentle volume and definition without the stiff, “done” look of heavy HA lip filler. Because it integrates with tissue, it moves as your lips move. Softens smile and lip lines. It rebuilds thinning skin along the vertical “barcode” lip lines and the smile lines framing the mouth, refining texture and softening creases that deepen with age. There is real evidence behind an ECM approach here. A 2025 double-blind, multicentre randomised controlled trial in Aesthetic Plastic Surgery found a micronised acellular dermal matrix filler was non-inferior to an approved collagen filler for correcting moderate-to-severe nasolabial folds — supporting the use of human-derived matrix in exactly this perioral region. How Re20 works: three mechanisms, one material Re20 Fine doesn't just sit under the skin. Its ECM material renews the skin through three complementary mechanisms. 1. Scaffold. It provides a structural template your own cells can migrate into and rebuild upon — physically restoring the framework thin skin has lost. 2. Signal. The matrix acts as a biochemical communicator, prompting resident fibroblasts to switch back on and produce fresh collagen and elastin. 3. Mechanotransduction. Restored structure lets the skin translate physical movement into healthy biological response again — the way youthful, resilient skin does. Crucially, this is remodelling, not reaction. Because Re20 is real human-derived matrix, your body recognises it and integrates it through a natural, host-mediated pathway. Preclinical research published in ACS Applied Bio Materials (2025) showed a micronised acellular dermal matrix integrated into host tissue with fibroblast infiltration and new blood-vessel formation, and only minimal inflammation — because the processing removes the cellular DNA and immune-triggering MHC proteins. There is no synthetic implant left behind to wall off. Results build gradually, the way real skin renews. A subtle refreshed look appears within days; texture and fine lines refine over 2–4 weeks; firmness, brightness and dark-circle improvement continue over 6–12 weeks as your own collagen returns. A 2026 human application study reported measurable improvement across multiple skin indicators — density, elasticity, barrier function, hydration and fine-line depth among them — with effects continuing to develop over roughly 20 weeks. The ideal combination: the RE-NEAU™ protocol Re20 Fine rebuilds the foundation. Pair it with a low-swelling PEG-HA filler for immediate, precise volume correction, and you have our RE-NEAU™ under-eye protocol — Dr Park Ji-Youn's proprietary approach to tired eyes. The PEG-HA delivers early structural correction with minimal swelling; Re20 Fine rebuilds the scaffold and improves tone, so the result looks natural and holds longer. For deeper dark circles and fragile skin, an enhanced tier adds a brightening polynucleotide layer. As a rule, Re20 and Re20 Fine also complement energy-based skin treatments and anti-wrinkle injections — because a stronger ECM foundation helps the skin respond well to almost everything else. Re20 Volume and ECM: Shaping the future of anti-ageing If Re20 renews skin quality and Re20 Fine perfects the delicate zones, the logical next step is depth — and that is where Re20 Volume comes in. Launching as the next member of the Re20 line, Re20 Volume uses larger ECM particles engineered for deeper placement: structural filling, facial sculpting, lifting and volume restoration. The thesis behind it is worth understanding, because it may signal where the whole field is heading. Conventional volumising fillers fall into two camps: hyaluronic acid, which adds water-holding gel, and biostimulators such as CaHA (calcium hydroxylapatite) and PDLLA (poly-D,L-lactic acid), which prompt collagen by introducing a synthetic material the body eventually clears. Both, in a sense, are props you rent and return — temporary or foreign structures the body manages around. An ECM approach is different: it aims to be scaffolding that becomes the building — matrix your own cells colonise and convert into living tissue. This isn't only theory. Injectable, micronised acellular dermal matrix has already been studied as a facial filler: the 2025 randomised controlled trial noted above found it non-inferior to a collagen filler for nasolabial folds, and the 2025 ACS Applied Bio Materials work demonstrated genuine host-tissue integration rather than a walled-off implant. Taken together with the growing consensus — reflected in current ECM research and echoed at recent scientific meetings — that restoring the extracellular matrix is a final common pathway of skin longevity, the trajectory points toward ECM-based volumisers as a natural, tissue-integrating alternative to today's synthetic biopolymers. It would be premature to promise that ECM will replace HA, CaHA or PDLLA outright — the evidence base is still maturing, and every material has its place. But the direction of travel is clear: fewer foreign bodies, more of your own tissue. Re20 Volume will be assessed and offered on an individual basis; suitability and results always depend on your anatomy and goals. Is it safe? What to expect Side effects are generally mild and temporary: minor redness, small injection-point bumps or slight tenderness that typically settle within 24–72 hours. Light bruising is possible depending on your capillary structure. Because Re20 is human-derived matrix with the immune-triggering components removed, its biocompatibility profile is strong, and there is no synthetic implant to provoke a foreign-body response. The bluish Tyndall tint sometimes seen with under-eye HA is not typically associated with Re20 Fine at appropriate injection depth — and issues such as unevenness are largely technique-related, which is exactly why this should be performed by an experienced, registered medical doctor. Downtime is minimal; most patients return to normal activities the same or next day. Avoid strenuous exercise, saunas, steam rooms and alcohol for 24–48 hours. Essentially, yes — same human-derived ECM material, micronised to a finer particle (~50µm vs ~75µm). That finer grind is what makes it suitable for the thin, mobile skin around the eyes and lips, where a coarser particle risks visible bumps. Can classic Re20 be used under the eyes? It isn't the ideal choice. Under-eye skin is extremely thin and constantly moving, so a coarser particle is more likely to show as bumps or create irregularity. Re20 Fine was refined specifically for this zone. Which is ideal for lips, Re20 or Re20 Fine? Re20 Fine. Lips are thin and highly mobile, so the ultra-fine particle integrates and moves naturally with your lips — freshening, hydrating and subtly defining without a stiff, overfilled look. Does Re20 or Re20 Fine hurt? Most patients find it very tolerable. Numbing cream is applied first, and because the particles are fine, the material passes through tissue with less resistance. Many describe Re20 as around 2 out of 10 for discomfort. How long do the results last? Because Re20 improves the skin's own structure rather than only adding gel, results are progressive and durable. Following a recommended course, many patients maintain a firmer, revitalised result for around a year or more, with periodic maintenance. How many sessions will I need? Many patients are advised a short initial course spaced a few weeks apart to build the result, followed by maintenance. Some concerns respond well to a single session. This is personalised at consultation. Is Re20 a filler or a skin booster? It's an ECM skin booster — a distinct category. Rather than adding volume like an HA filler or stimulating like a biostimulator, it replenishes the skin's structural matrix directly. It can, however, be combined with a filler (as in RE-NEAU™) for volume plus structure. Will Re20 Fine cause the Tyndall effect or a blue tint? The bluish Tyndall tint sometimes seen with under-eye HA is not typically associated with Re20 Fine at appropriate injection depth. Correct technique and depth are key — another reason to choose an experienced doctor. Can I combine Re20 Fine with HA filler? Yes — this is often recommended. Re20 Fine pairs with a low-swelling PEG-HA filler in our RE-NEAU™ under-eye protocol, giving immediate volume correction plus long-term structural rebuilding. Is Re20 safe, and who performs it? Re20 has a strong biocompatibility profile as a human-derived, decellularised matrix, and is performed by registered medical doctors after an individual assessment. Suitability and results vary by person, which is why a consultation comes first. What is Re20 Volume, and when is it available? Re20 Volume is the forthcoming member of the Re20 line, using larger ECM particles for deeper filling, facial sculpting and volume restoration — an ECM-based approach to the work HA and biostimulator fillers currently do. Ask our team about current availability. The bottom line Re20 and Re20 Fine aren't rivals — they're the same ECM material in two grinds, each built for a different job. Choose Re20 Fine for the thin, mobile skin of the eyes and lips, where its ultra-fine particle settles smoothly and moves naturally. Choose classic Re20 for the broader canvas of the face. And watch for Re20 Volume, which extends the same tissue-integrating philosophy to deeper, structural work. As with any injectable, the material is only half the equation — the hands placing it matter just as much, especially on delicate under-eye and lip skin. The right result comes from matching the right grind to the right area, in the right plane, after a proper assessment.

The bottom line

Re20 and Re20 Fine aren't rivals — they're the same ECM material in two grinds, each built for a different job.
